Social Support Program
Funded by Department of Health (Vic), this is a friendly visiting program for the older and frail person from culturally and linguistically diverse backgrounds. Clients are matched with a volunteer appropriate to their culture, gender and age. Volunteers visit the client for social interaction. The service also provides occasional transport assistance, telephone monitoring and social and recreational activities. Activities can be for clients or volunteers. Support for Carers Program
Funded by the Department of Health, Aged Care, this program provides support to carers from a culturally and linguistically diverse background. Our target is either (i) Older people who are carers themselves, of someone younger with a disability, or (ii) People who are caring for an older person.
This program provides flexible support, information and referrals to family carers to support them in their caring role. We provide monthly information and support sessions, and monthly outings. Some of our activities also include care recipients. All activities aim to maintain the social, physical and emotional health of the carers themselves. We encourage carers to take appropriate respite. We aim to assist all who seek our assistance; however our priority is for carers needing the greatest support or those without other family help.
Access and Support Projects
SMRC has a range of projects which aim to improve access to a wide range of aged care services, especially Home and Community Care. This suite of programs provides assistance to frail older people, adults with a disability and family carers. Team members provide short term support to those individuals who face barriers to accessing appropriate age care services and community education to groups. We work closely with service providers to enhance the cultural responsiveness of their service and to reduce the cultural and linguistic barriers facing the ethnic community. Projects include: Support and Access and HACC Service System Resourcing programs funded by the Department of Health (Vic), and also, a Service Improvement and Healthy Ageing project. Funded by the Federal Department of Social Services, this program works with Residential Aged Care services.
